Endodontic microsurgery - root resection

Root resection involves amputating the apex of a tooth's root and removing lesions within it. The apex resection procedure is performed in the case of inflammatory periapical lesions that do not resolve after root canal treatment (they are visible on imaging) or if the tooth continues to give symptoms of pain.Resection is also performed when the apex of the root has broken or perforated (known as perforation) or when a dental tool is stuck in it. Still another reason for the procedure may be an unusual anatomical structure of the tooth that prevents effective root canal treatment, such as a significantly curved canal. Most often, the resection procedure is performed in teeth restored prosthetically with a crown and root-canal post. It is then a good alternative to destroying, often expensive, prosthetic work.

After getting into the area of the root apex, the doctor removes inflammatory tissue and the root apex of min. 3 mm (no more than 1/3 of the total length), retrogradely develops the canal, fills it with the material intended for this purpose - MTA, and places sutures, which are removed after about five days. During the procedure, a final assessment is also made of the possibility of preserving the tooth - ruling out vertical tooth cracks or fractures.
